Complete the Heart Art order form for each piece of artwork. Paperclip the form to the artwork, write the child’s name on the back of the artwork in light pen and place (unfolded) into envelope.

Drop off artwork at select drop off locations or contact Carol Hager at Heart Art to mail in or make other arrangements.

Upon your approval, via email, both the original work and the completed Heart Art can be picked up at the same drop off location or mailed to you in a protective cylinder tube. Shipping not included.

There is no minimum size requirement for the original artwork. Maximum size is 11” x 17."

**Each piece of art is $50 plus tax and each additional reprint is$25 plus tax.  Download Order Form
